Does your drive click or make strange noises when attempting access to LBA ? Can you read ANY LBA at all ?

Problem might be firmware related.

Strange report from your firmware tool ....

If the drive is clicking non-stop better STOP messing with it and outsource the drive to someone with better tools and more experience ....

When you plug the drive for example to a IDE 2.5 PC3K adaptor and you power it on without sending any ATA command does the drive click as well ?

Drive firmware is on platter. Your tool does report accessing to drive firmware OK so maybe it did read something ? Strange ... Maybe a bad head ? More testing with proper tools would be required ...

Did you make any progress with this ? Maybe you have some sort of firmware damage as well ?
