I have this error with this Toshiba Transmemory 16GB USB.
VID:04B3 PID:3025
I have successfully flashed a Kingston that had the same error using Phison UPTool but that would only leave the drive "usable" erasing everything.
The I/O refers to the controller, right? I could "just" do a chip-off recovery.

yes for IO error, either fix PCB problems (resistor short/open or broken solder joints) or chip-off.

As per Chipgenius instructions, disconnect all other things usb from your computer before running it. It probably is detecting your keyboard, or you really have a fake usb drive.

When controller goes bad, anything can be reported. regardless of what it is reporting, just read the chip.
is it a monolith or what is the controller and chip? what did CG say the nand ID is?
