Samsung Galaxy 5s Active formated SD XC card not formatting

Help! Just got a new Galaxy S9+, went from a Samsung 5s Active,...where I used a SD XC 128gig card for music & photos/videos, that worked great for years. In trying to erase & reformat from the old phone....it wont reformat or even delete content, LOCKED in!

Just tried a low level format, (from http://hddguru.com/software/HDD-LLF-Low ... rmat-Tool/,) - which just failed as well. Tried Mac BookPro utilities, Windows formatting- again no luck.... When I put it in the new phone it continually says its corrupted, but when I put it in my desk top (Win 10) I stil see all the content from the first phone.... I am stumped. Suggestions? Is the 5s phone still working? If yes, try to format there and then test back on the S9.

It is... but local AT&T has it... trade in on new phone.... I was thinking of that .... thanx

failed controller, set to read only and probably capacity issues. THe SD card is toast to 99% but see if this tool works https://www.sdcard.org/downloads/formatter_4/
