What's the possible cause of all these?

Is there something completely wrong with the head? Because no disk itself would be damaged like that.

You should scan more and see if there's a visible pattern in errors

ok good advise, I'm definitely gonna try it, problem is, even there is a pattern, I don't know what to do :)

I suspect at some point the acuator head is dissynchornized (hope you know what I mean) with the disk.

Then the head should be changed
