Is there something completely wrong with the head? Because no disk itself would be damaged like that.

You should scan more and see if there's a visible pattern in errors

ok good advise, I'm definitely gonna try it, problem is, even there is a pattern, I don't know what to do

I suspect at some point the acuator head is dissynchornized (hope you know what I mean) with the disk.

Then the head should be changed
