Can't read ID, get DRD, DSC after 10 sec. I have access to terminal.
Heads or SA problem?

Any hints will be helpful.

Can be both (weak head) or fw.

If problem is with FW how to check/solve it?

Patient heads work perfect in donor. So problem is with SA. Probably bad blocks in SA.
Friends, do you have way to deal with that?

What platform do you have ? (SD, PC3000, etc.) ?

PC3000.

What's the terminal log after head swap? The same?

I check only patient heads in donor. Works perfect, so I exclude problem with patent heads.
