Could be many things.
Bad sectors, bad heads, bad power supply, bad motherboard, power surge, virus, etc...

If you've got no malware and have all the relevant drivers installed correctly, then considering SMART (iltraDMA errs), most often it's an unstable power supply or a damaged data cable.
