endless problem with my lenovo y510p hdd, please help

for the past 3 weeks iv tried on my on to overcome this annoying problem.
i dont want to give up and buy a new hdd, the computer is only 2 years old.

i had a legit copy of windows 8, one day start to make startup problems, and give me this kind of errors: "scanning and reparing drive {\\?\volume{953762cb-b71e-...

the error stay the same every time i try load the windows, after couple of days it gave another error: RECOVERY, your pc need repair, error code:0xc00000f...

i get its a hdd problem, iv give up on my legit copy of windows and start to format to a new coy of windows 8 from a bootable usb, after changing to lagacy support iv been able to install the new windows which i am currently use.
iv also open my pc and disconnect and reconnect my ram memory and the hdd.

problem now that my pc is not stable, sometimes become black screen and i have to turn off and on and again i get the same error of the {\\?\volume{953762cb-b71e-... but this time after about 5 minutes to windows is up and running, until the next time it fail and become black.

please what can i do to solve this problem? sorry for my English, thanks for the helpers

Can you upload here the SMART report ?
You can use http://crystalmark.info/download/index-e.html or whatever software you like )

hi jermy, im am origanly from israel by the way

i have no idea what is smart report and how to use it

i bought this lenovo from ksp in israel

Download and launch crystaldiskinfo from the link provided above and you'll have the SMART report in the second half (bottom) of the crystaldiskinfo window
