HDD "Health Status: Bad" Problem

So, I recently got this http://imgur.com/a/9SUUV pop-up from windows saying that my disk was about to fail soon and that I should create a backup immediately before my data is lost. This warning was about my C:/ drive so I backed up all my data and then downloaded CrystalDiskInfo only to be greeted by this image: http://imgur.com/a/zo6yq. Apparently my 10+ year old HDD isn't doing to well. Does this mean it's going to stop working soon? What does "Reallocated Sector Count" really mean? Thanks in advance.
