Second character of the ser. number shows the number of heads. There are 2-head and 3-head drives of this model.

True, but also there are and other place where you may understand this.
Also i think you are a bit wrong. 6Y0800L0 have 2 and 3 heads model
6Y120L0 have 3 and 4 heads models.

Maybe you don't need any...
Try first to find out which module is damaged. Maybe the second copy is good...

Bad disk module.

Module 00080001.rpm is damaged - Disk passport. Copy 2 is OK so you can read 3e4005dc.smb , cut the first 512 bytes and put them in file named 00080001.rpm (it should start with DISK). Write the module to the SA of the drive. Turn off / turn on. Voila.
