If 5V and 12V at present at the input, but one or the other is not present at the output, then the chip is faulty or the corresponding supply is shorted to ground.

thank you for your time responding Sir.i cant thank you enough
iv swapped the e-fuse from healthy donor board! the e-fuse of broken board work fine on donor board Sir may the smooth chip?
