This can be caused by week/dying heads but on your case as you apear to state that the relocation sector value doesn't increase there are some chances for a pcb issue (very rare) to be causing the ecc errors and so the drive decide to put the sector as pending because it thinks the sectors might be bad but then the sector is re-written it's found to be good.
In any case there is no simple way to fix a drive like this. Might involve attempting a PCB swap with ROM swap to be sure that pcb is not making that symptom (and most likely isn't) and even a BURN IN with expensive tools might not be reliable and might kill the drive.
So i would say just buy a new drive if you don't need the data.

I didn't want to buy a new drive in the first place because I use the desktop very rarely these days. I didn't want to invest on the machine anymore. So I thought if I could somehow make the drive work I'd go with that. I wouldn't put anything important on it anymore. Maybe I'll just use it as an entertainment hub.
I was finally able to zero-fill the whole drive using Maxtor PowerMax v4.23. It's surprising that Samsung's ESTool failed the job on it's own drive.
Anyway, after zero-filling I checked SMART status from Linux Mint live OS and it reported 'okay' for now - no current pending sectors or reallocated sectors.
I also ran short DST and this time it passed the test.
Now, how can I check the integrity of the drive at this point? I haven't installed any operating system on it yet.
