My HP 128GB pendrive is unable to format. i have used cmd prompt, basic format option in the windows, in the disk management, even several third party application such as HDD Low Level Format Tool 4.50, minitool partition, disk drill etc. but failed to format.

HDD LLF tool 4.50 says 'format error occurred at offset 6,225,920: 121- Device not ready (Timeout);
'format error occurred at offset 6,488,064: 1117- Device I/O error'.

How to format the usb pendrive? if anyone knows, please guide.
