dont know without further analysis. if you seek to recover data from drive to another destination a product such as R-Studio will scan the drive for what was there and allow you to recover the files. for discovering what did not happen there are utilities on this forums root that will allow you to read the condition of the drive. If drive tests perfect then the answer to your question would be 'outside forces- generally user caused and infrequently malware caused. if ssd may be some other possibilities. too little info to continue guessing...

free tools here on the same website to check if drive is mechanically sound you are on right now ( http://www.hddguru.com/software ) are mhdd victoria hddscan ... btw- dont diagnose drive to death- start with quick SMART read... if failing sectors back off--- time to use a cloning tool that handles that issue. an inexpensive favorite of mine for deep file recovery is r-studio ( http://www.r-studio.com ) I set it to scan and recover files by the type I am hunting for- very well done product.
