The error Flash connectivity test fail (22)/(08.12) means the tool failed during a flash connectivity test — essentially, it tries to communicate with the NAND dies through the controller’s internal channels and CEs (Chip Enables), and at some point it didn’t get the expected response.
Relevant bits from your report:

Controller: SM2259XA bufferless
FlashID: 0x2c,0xd3,0x0c,0x32,0xea,0x30,0x0,0x0 — Micron 176L QLC
CE: 1, Channel: 2, TotDie: 2
Bad Block From Pretest: 74
Original Spare Block Count: 96
Possible reasons for the failure:

High bad block count:
You already have 74 bad blocks before any operation. That’s quite a lot for a 240GB QLC drive. On QLC NAND (especially Micron 176L), a higher bad block rate can cause issues in connectivity or read/write mapping.
Incorrect or incomplete flash mapping table (FEMU config) for this specific NAND
If the firmware or flash map config you’re working with doesn’t fully support this NAND or has mapping assumptions not matching your CE/chip/die layout, connectivity tests can fail.
Controller quirks — SM2259XA bufferless is somewhat finicky
This variant sometimes behaves differently in factory tools versus older SMI tools. Connectivity test patterns may expect specific CE toggling patterns or plane interleaves.

What you can check/do:
Verify the FEMU / mapping table you’re using matches this NAND config
1 CE
2 Planes
1024Gb per Die
Some factory tools expect 2 or 4 CEs for similar capacity, and if misconfigured, a connectivity test will fail.
Check if your flash config file (if using one) has the correct Flash ID entry for 0x2cd30c32ea30
(you might be using a config with 64L or 96L assumptions instead of 176L QLC)
Try bypassing the connectivity test if possible in your tool
Some versions of SMI tools allow continuing even if that test fails, though of course it may cause problems later if the issue is deeper.
Double-check your NAND’s physical soldering or connection (if DIY modded)
Intermittent connections or CE line issues could trigger connectivity failures, though less likely if you didn’t rework the drive.

Can you make a short video with the steps you take up to the error message?
